How to Apply to Use GCRC Resources
In an attempt to reduce redundancy when submitting a proposal to the GCRC, we have streamlined the application process by having a combined IRB/GCRC application. In addition to completing the IRB application, please be sure to include the following documents when requesting GCRC resources:
1. Appendix A,
Appendix B
and Appendix D. These appendices can be found in the IRB public folder in the IRB/GCRC application folder. Appendix A pertains to the specific resources you are requesting from the GCRC. Appendix B is the Data and Safety Monitoring Plan. Appendix D describes recruitment goals.
2. A detailed research proposal. The proposal must include: specific aims, background and significance, methods, and references. If the proposed study has already been approved for funding by NIH, a Foundation, or HCRAC, or as an investigator-initiated industry study, the final proposal used to obtain funding can be submitted.
3. Reviews/critiques from the funding agency. Please provide reviews/critiques (i.e., summary statement, if NIH) from the funding agency supporting your project, as well as your response/rebuttal. If this is not available at the time of the GCRC application, please indicate that in a cover memo and forward the material once it becomes available.
4. Budget from funding agency. Please provide a copy of the approved budget and budget justification from the funding agency. If this is not available at the time of the GCRC application, please indicate that in a cover memo and forward the materials once it becomes available.
5. PI Biosketch. An NIH Biosketch is required.
6. Supporting Documentation. Approved Informed Consent Forms and HIPAA Authorization are also required. If these are not available at the time of the GCRC application, please indicate that in a cover memo and forward them once they become available.

Changes To Protocol
Use this form for any changes to your existing approved and ongoing protocol. (This includes additions, deletions, exceptions to enrollment, protocol revisions, etc.)
